New Client Forms Instructions

Once you have scheduled an appointment with me you can fill out these forms and either bring them in at your first appointment or email them to me.  This will save the time of filling them out in my office.

• Adult clients should fill out the forms labeled Client Information and Informed Consent.
• Couples should fill out the forms labeled Couple Information and Informed Consent Couple.
• Parents of minors should fill the Child information and Informed consent minor forms.

All clients should read and sign the Notice of Privacy Practice.  It is in your best interest to understand my counseling procedures as well as the protections and limits of confidentiality.
